Request for Proxies for upcoming AZM on 15th October 2022

As current MAAC members, you are aware of the upcoming Annual Zone Meeting this Saturday a week (15th October 2022).

I am soliciting Open Proxies for the meeting please. This is to ensure that we a) Meet the Quorum Requirements of 71 members attending in person or by proxy b) to enable us to vote on the Agenda and items on the agenda.

You may type in your name in lieu of your signature and when completed, please email to me,  as soon as possible but no later than the 14th October 2022.
You may also complete the form, sign and scan or photograph the completed form and then return to me.

Please complete the attached Open Proxy assigning your vote to me and return it to me via email to ([email protected]).
